OVERVIEW
Canthoplasty and canthopexy are procedures that tighten or reshape the outer corner (lateral canthus) of the eyelids. Canthopexy reinforces the eyelid's lateral support without cutting the tendon, while canthoplasty involves a more significant reconstruction of the lateral canthal tendon. Both procedures address eyelid laxity, reshape the eye opening, and are often combined with blepharoplasty.

AM I A CANDIDATE?
- Patients with lower eyelid laxity, visible white below the iris (scleral show), a rounded or sagging outer corner, or those undergoing lower blepharoplasty who need additional eyelid support are candidates. A canthoplasty or canthopexy may also be appropriate for patients seeking to achieve a specific eye shape such as an upswept lateral canthus.
THE PROCEDURE
In canthopexy, a permanent suture is passed through the lateral canthal tendon and anchored to the orbital rim periosteum, providing improved support without detaching the tendon. In canthoplasty, the lateral canthal tendon is detached, resected to remove laxity, and reattached to the orbital rim at the desired position and tension, fully reconstructing the lateral canthus.
